Does a concsious speak as an audible voice?

I've read the Bible and in Romans and Hebrews it says that a conscience can condemn a person or it can also be evil. How does one hear a concious and why would it be evil or want to condemn? Does it sometimes work contrary to what the Bible teaches about forgiveness?
